Nureli Medical Group, PLLC (“Nureli Medical Group,” “Nureli,” “we,” “our,” or “us”) is committed to protecting the privacy and security of your health information. This Notice of Privacy Practices (“Notice”) describes how we may use and disclose your protected health information (“PHI”), your rights regarding your health information, and our legal obligations under applicable federal and state privacy laws, including the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).

Accounting generally does not include disclosures related to treatment, payment, healthcare operations, or other disclosures excluded by law.
Right to Request Restrictions
You may ask us to restrict certain uses or disclosures of your protected health information for treatment, payment, or healthcare operations.
We are generally not required to agree to your request unless otherwise required by law.
If you pay for a healthcare service or item entirely out of pocket, you may ask us not to disclose information about that service or item to your health plan for payment or healthcare operations purposes. We will honor such a request when required by law.
Right to Request Confidential Communications
You may request that we communicate with you about your health information in a specific way or at a specific location.
We will accommodate reasonable requests as required by law.
Right to Receive a Copy of This Notice
You have the right to receive a paper or electronic copy of this Notice at any time, even if you previously agreed to receive the Notice electronically.
Right to Choose Someone to Act on Your Behalf
If you have given someone medical power of attorney or if someone is your legal guardian or authorized personal representative, that individual may exercise certain rights and make choices regarding your health information as permitted by law.
We may verify that the individual has the appropriate legal authority before taking action.
Your Choices
For certain health information, you may tell us your preferences regarding what we share.
In certain situations, you may have the right to direct us regarding the sharing of information with family members, friends, caregivers, or others involved in your care or payment for it.
If you are unable to communicate your preferences, we may use our professional judgment and applicable law to determine whether sharing information is in your best interest.
Specially Protected Information
Certain categories of health information may receive additional protections under federal or state law. These may include, depending on applicable law and the services provided, information related to mental health, psychotherapy notes, substance use disorder treatment records, HIV/AIDS status, genetic information, reproductive healthcare, and other sensitive health information.
Nureli Medical Group will comply with applicable laws governing the use and disclosure of specially protected health information.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with Nureli Medical Group.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
We will not retaliate against you for filing a complaint or exercising any of your privacy rights.
Changes to This Notice
We reserve the right to change the terms of this Notice and our privacy practices as permitted by law.
Any revised Notice may apply to protected health information we already maintain as well as information we receive in the future.
The current version of this Notice will be available on our website and upon request.
Website Privacy and Patient Communications
The Nureli Medical Group website is intended to provide general information about our organization, healthcare services, providers, and availability of care.
Nureli Medical Group does not use this website to collect protected health information (“PHI”) or patient clinical information. Please do not submit personal health information or sensitive medical information through the Nureli Medical Group website.
Links provided on this website for patient communication, appointment requests, scheduling, or other healthcare-related communications may direct you to secure third-party platforms used by Nureli Medical Group for patient communications and healthcare operations. Patient communications initiated through these platforms are handled within the applicable secure platform and are subject to Nureli Medical Group's privacy and security practices.
The Nureli Medical Group website and our secure patient communication systems are separate systems. Information submitted directly through an authorized patient communication platform is neither submitted to nor stored by this website.
If you have a medical emergency, call 911 or seek care at the nearest emergency department. Do not use the Nureli Medical Group website or non-emergency electronic communication methods for urgent or emergency medical needs.
